Google Form- Project Proposal Sheet

I started using Google Forms for my students to submit their weekly Project Proposal sheets. I love it. It allows me to print a spreadsheet of all the projects going on in my room at one time. I downloaded the spreadsheet to my iPad for quick updates as I walk around the class. I'm working on a reflection form for students to fill out at the end of the week.

This is what the form looks like from the student perspective:
